To determine the incidence and clinical features of Kawasaki syndrome (Kawasaki disease) in Trinidad & Tobago. We conducted a retrospective study of all patients diagnosed with Kawasaki syndrome over a seven-year period between January 1995 and December 1999 at the Royal Victoria Hospital, Port of Spain, Trinidad & Tobago. There were 20 cases of Kawasaki syndrome; 10 male and 10 female. The incidence was 0.6 per 100 000 population over the study period. The highest incidence of Kawasaki syndrome was in October through December each year. Kawasaki syndrome was diagnosed on clinical grounds with the following findings; fever for more than five days, splenomegaly, bilateral conjunctival injection, oedema of the hands and feet, cervical lymphadenopathy and rash. All patients treated with intravenous immunoglobulin had a favourable outcome with complete resolution of clinical and laboratory features of Kawasaki syndrome within seven days of treatment. The clinical features of Kawasaki syndrome are similar to those described in Japan and American children. The observed incidence of Kawasaki syndrome in this study is higher than previously reported.Botulinum Toxin A (BTA) therapy is generally effective for the treatment of a number of conditions but studies have demonstrated a decline in efficacy in BTA treatment of cervical dystonia. While additional studies will be needed, the differences in muscle relaxant effects in dystonia may be due to the great variation in the nature of dystonia.
